Lost Cards

Policy:

Edit the existing patron registration work form to replace the barcode with a new barcode following the procedures in the Patron Services section of the Polaris Training Manual.  You may not check out materials using a previous patron barcode.

If a LOST card should ever be passed to a library staff member by a patron who intends to check out items with it--please take this card from the patron and destroy it.

Recommendation:

When replacing a lost card the old barcode should be copied into the “Former barcode” field in the General view of the patron registration work form. If there is already a former barcode listed in this location, overwrite it.

The charge for replacing a lost library card should be $3.00.

To eliminate confusion, patrons should be reminded to update their barcode on all personal devices. 

Procedures:

If the patron uses eBCCLS, use the Patron Barcode Change Support Form found on the Support Form page of the BCCLS Staff website to report the patron's barcode change. This will enable BCCLS staff to migrate any eholdings from the patron's previous to current patron IDs.

  • Procedure for Changing a Patron’s Barcode for Active Cloud Library Users

1. When changing a patron barcode, go to the BCCLS Staff page and select Support Tab.
2. Select Support from the drop down menu, and under Support Forms select Problem Type: Patron Barcode Change.
3. Fill in the required fields: Patron Name, New Barcode, Original Barcode, and select if patron has any eBooks or eAudiobooks out.*
4. Click Send to Help Desk. A copy will go to BCCLS Office and the library issuing the original barcode.

*If the patron is unsure if eContent is checked out, items can be viewed through the patron’s PAC account.
